I am trying to find a way to keep track of all the medical tests/results, medications, dates of past procedures as well as current issues or side effects of new medications.

Knowing me, an iPad app would work best. Currently I use Noteshelf and am thinking about creating a separate journal for this information.

What works for you?

There's an iPhone app called GenieMD that a lot of our patients are using. There's probably an iPad version, too.

I have a binder for each of us now. I chose not to do the iPad as if the doctor needs a copy of something they often aren't equipped to have me print it to them. I do have everything scanned though in case binder is misplaced.

My medical provider is the VA. I can walk into any VA hospital or affiliated clinic and they have my medical records on line. My meds, eye exams, past operations, my disability history, everything.
At home I have a chart to show my meds, and my PCP phone & address, blood type and things a fire fighter wants to know. It is in a plastic envelope attached to the refrigerator with magnets clearly marked EMT INFO. One of my sons is a fire medic.

I keep one for each of us. I am diabetic and take high blood pressure meds. Plus DH has meds and treatments for his Bipolar. the kids have basic medical information going back to birth plus any meds they are taking. It help me take care of things when they all get sick.

How mine is set up
doctors information
medication list (up dated anytime something is add)
Appointment's list
Blood Glucose and Blood pressure numbers
Test
Medical notes I take in to the doctor.

i keep them all up to date and ready to go. I saves so much time in the hospital. i have a copy of most of the 5 on a basic zip drive if i need it.



I have quite a few learing disabilitys that effect my spelling a grammer. I do know my grammer and spelling suck. I have been working on this problem all my adult life.

I use a USB bracelet similar to these. It has a program that includes all medical information AND you can store a DNR/Living Will on it as well. These things are amazing AND universal since you can just plug the USB into a computer and ALL of your information pops up.

It also includes a note area along with the med list, procedure list, hospitalizations, etc.
