I always thread them - either sewn on the layout or with thread/twine through and tied with a knot at the bakc or a bow at the front.

Since most of my buttons are quite plain and boring I like the extra colour and detail it gives. If I had more patterned buttons I would probably thrad them less often.

NOT. I think they somehow look "cleaner" that way. When I see them threaded it always reminds me of "Shabby Chic" and that's just not my style.

Depends on how I am using them. If I am using a few in clusters I will use thread in them. Or floss.

If I am using a lot of buttons in a row I don't. I did a LO using about 100 buttons in two waves across the LO. I didn't thread those
